It is a fair question, and I want to give you a genuinely useful answer, not a sales pitch for one brand over another, but a real breakdown of what separates Motiva, Natrelle, and Mentor from each other, what the research and clinical experience actually show, and how I think about implant selection for my patients in Dallas, Plano, Southlake, and across the DFW area.
The short version: all three are excellent, FDA-cleared implant brands with strong safety records and extensive customization options. The best implant for you is not the best implant in the abstract. It is the one that fits your anatomy, your lifestyle, your goals, and your surgeon’s recommendation. Mentor is another long-established implant brand with an extensive clinical history. Also FDA-cleared for both saline and silicone gel options, Mentor is known for its MemoryGel line of silicone implants, which use a cohesive gel formulation that retains shape while maintaining a soft, natural feel. Mentor’s MemoryGel Xtra offers a slightly firmer gel for patients seeking more shape retention, and their CPG (Contour Profile Gel) line provides anatomical options. Like Natrelle, Mentor implants are well-studied, widely available, and carry a strong safety profile supported by extensive long-term clinical data.
Motiva (Establishment Labs)
Motiva implants, manufactured by Establishment Labs, represent the newest generation of the three brands discussed here. They are 6th-generation silicone implants built around three proprietary technologies: Ergonomix2® adaptive shaping technology, SmoothSilk® nano-surface engineering, and ProgressiveGel® fill. Motiva implants are FDA-cleared for primary breast augmentation in women 22 and older and for revision surgery.
Motiva’s key differentiator is positional adaptability. Ergonomix2® implants shift dynamically with body position, forming a teardrop shape when standing and a softer, rounder contour when lying down, closely mimicking the behavior of natural breast tissue. This dynamic responsiveness, combined with the SmoothSilk® surface designed to reduce capsular contracture risk, is what makes Motiva a compelling option for patients prioritizing the most natural possible result.
Motiva vs. Natrelle vs. Mentor: Key Differences
Feel and Natural Movement
Motiva earns consistent praise from surgeons and patients for producing the softest, most natural-feeling result of the three. The Ergonomix2® technology and ProgressiveGel® fill give Motiva implants a dynamic, tissue-like quality that neither Natrelle nor Mentor fully replicates. For patients who put natural feel and movement at the top of their priority list, Motiva has a meaningful advantage.
Natrelle and Mentor both offer excellent, soft, natural-feeling results with their standard silicone gel lines. The difference is primarily that they do not adapt positionally the way Motiva’s Ergonomix2® implants do. In practice, most patients are very happy with the feel of both Natrelle and Mentor silicone, particularly with softer gel options like Natrelle Inspira or Mentor MemoryGel.
Capsular Contracture Risk
Motiva has invested significantly in reducing capsular contracture risk through its SmoothSilk® nano-surface engineering. The surface is designed to encourage favorable tissue integration and reduce the low-grade inflammatory response believed to trigger capsular contracture. Early data and real-world clinical experience from surgeons using Motiva are encouraging, though long-term population-level data of the same depth as Natrelle and Mentor is still accumulating given that Motiva is the newest of the three brands.
Natrelle and Mentor both have extensive long-term data on capsular contracture rates across their implant lines. Dr. Abtahi’s Keller Funnel no-touch insertion technique significantly reduces contamination risk and capsular contracture rates with any implant brand.
Implant Options and Customization
Natrelle and Mentor offer the broadest range of customization options, including saline, standard silicone gel, highly cohesive silicone, and anatomical (shaped) implant options. Motiva offers Round and Ergonomix® lines in a wide range of sizes and profiles, comprehensive for the majority of patients but narrower overall than the full Natrelle or Mentor catalog. Motiva does not currently offer saline implants.
Clinical Track Record and Data
Natrelle and Mentor have the longest clinical track records, with decades of large-scale outcome studies, FDA clinical trial data, and real-world experience across millions of patients.
Motiva is the newest of the three and has a shorter track record in the U.S. market. Its FDA clearance is more recent, and while clinical data are positive, they are not yet as extensive. This reflects the brand’s shorter U.S. market history, not a safety concern.
Cost
Motiva implants carry a higher per-implant cost than Natrelle or Mentor, reflecting the proprietary technology and newer manufacturing process. Natrelle and Mentor are generally priced similarly to each other and typically somewhat less expensive than Motiva. The right implant for you should not be chosen on cost alone, but it is a realistic factor worth discussing during your consultation.
A Note on Textured Implants and BIA-ALCL
If you have been researching breast implants, you have likely come across news about BIA-ALCL (breast implant-associated anaplastic large cell lymphoma), a rare immune system cancer linked to textured breast implants. BIA-ALCL has been associated with macro-textured implant surfaces. In 2019, Allergan voluntarily recalled its Biocell textured implant line. Mentor’s SILTEX textured line has not been recalled but carries an FDA warning. Smooth implants from all three brands, including Motiva’s SmoothSilk® surface, have not been associated with BIA-ALCL.

- Preferences on data: some patients are more comfortable with implants that have decades of outcome data behind them. I respect that and will never pressure a patient toward a newer brand
- Budget: I will always be honest about cost differences and help you make a decision you feel good about at every level
The implant is one important variable in a breast augmentation outcome, but it is not the only one. Surgical technique, incision placement, pocket creation, and implant insertion method all matter enormously. A well-placed Mentor implant in the right patient, inserted with the Keller Funnel no-touch technique, will consistently outperform the “best” implant placed with poor technique. Choosing the right surgeon is at least as important as choosing the right implant brand.

Choose Natrelle if you want the widest customization options including anatomical implants, decades of outcome data, and a proven track record across millions of patients. Natrelle’s Inspira line offers excellent softness and feel for patients who want a natural result without the Motiva premium.
Choose Mentor if you prioritize long clinical history, comprehensive size and profile options, and a well-established safety record. Mentor’s MemoryGel silicone is consistently well-reviewed for its natural feel and durability.
