Facelift FAQs
Most patients feel comfortable returning to work or social obligations 10 to 14 days following their facelift and neck lift surgery. At a week with appropriate camouflaging (hair down, scarf, sunglasses, etc.), you can easily go to the grocery store or small outing without drawing much attention to yourself. On average, in one week, about 50% of your swelling will be gone. At 2 weeks, about 80% of your swelling will be gone. At 2 months, about 90% of your swelling will be gone. Adjunctive procedures, like fat grafting, eyelid surgery, lip lift, brow lift, or CO2 laser, may extend this timeline.
If you ask 5 different surgeons what a mini facelift is, you’ll get 5 different answers. Rather than use this nomenclature, Dr. Abtahi prefers to simply describe the areas of the face being lifted. A traditional facelift involves addressing laxity of the cheek, lower face, and neck. A mini facelift addresses only one or two of these components. In both cases, the technique used is the same – a deep plane approach where ligaments are released for a natural and maximized lift. In some cases, a minimal access approach may be used to lift all the areas that a traditional facelift does. Every facial surgery case is different and is customized to the patient’s needs.
? During your pre-operative visit, you’ll receive detailed instructions, a list of medications to avoid, post-operative care guidelines, necessary supplies, and prescriptions. Fill prescriptions before surgery day and share instructions with your caregiver. Meal prepping ahead of time is also helpful.
? Facelift patients range widely in age. Weight loss patients may seek surgery in their 20s to 30s, while those in their 40s often pursue lifting to achieve subtle results and avoid overfilled appearances. The most common age group is 50s to 60s. Dr. Abtahi also performs facelifts on patients in their 70s and 80s. Any healthy adult with a correctable condition is a potential candidate.
A “liquid facelift” is a misnomer since dermal fillers do not lift tissue. Filler simply adds volume and improves the contour and proportions of the face. This is easier to get away with when you are younger, but as you age and require more product to contour, overfilling is a risk and mistake made by many non-surgeon practitioners. Unfortunately, the alienation of the face by overfilling is what many patients assume is a surgical outcome. A traditional facelift or neck lift surgery will make you appear more natural as it is simply repositioning and lifting your tissues to where they came from when you were younger.
Most modern facelift surgery involves tightening the layer of tissue known as the SMAS. During a deep plane facelift, Dr. Abtahi elevates this layer of tissue in the face and releases the ligaments in the face to allow for maximal lifting power and durability as well as the most natural repositioning of the aging tissues. A deep plane facelift also eliminates skin tension once the skin excess is removed and tailored. The deep plane facelift is an advanced procedure, and most surgeons do not utilize this technique because of its complexity.
Yes. Incisions are designed to allow hair growth through them along the hairline, while remaining incisions are placed in natural facial creases for optimal camouflage.
Most patients feel comfortable returning to work or social obligations 10 to 14 days following their facelift surgery. On average, in a week, about 50% of your swelling will be gone. At 2 weeks, about 80% of your swelling will be gone. At 2 months, about 90% of your swelling will be gone. Adjunctive procedures, like lip lift, blepharoplasty, fat grafting, or CO2 laser, may slightly prolong this timeline.
Good candidates have moderate to significant facial sagging, jowls, deep folds, or loose neck skin, are in good overall health, are non-smokers or willing to quit, and have realistic expectations. Most patients are 40-70 years old, though candidacy depends more on the degree of facial aging than age itself.
Results typically last 10 to 15 years, varying based on genetics, lifestyle, sun exposure, and skincare habits. While your skin continues aging, you’ll always look younger than if you hadn’t had surgery. Some patients choose revision lifts after 10 to 15 years, while others enjoy lasting results for 15 to 20 or more years with proper maintenance.
You should be at or near your goal weight and maintain stable weight for several months before surgery. Facelifts refine facial contours in patients already at a healthy weight. Significant weight loss after surgery can cause renewed sagging, while weight gain can stretch tissues and compromise results.
Yes. Male facelifts require specialized techniques to preserve masculine facial characteristics, natural hairline and beard patterns, and sideburn positioning. Male facial anatomy differs from female, so it’s important to choose a surgeon highly experienced with male facelifts like Dr. Abtahi.
Neck Lift FAQS
A neck lift, or lower rhytidectomy, addresses sagging skin, excess fat, and muscle laxity in the neck area. The procedure removes excess skin, tightens underlying muscles (platysma), and may include liposuction to eliminate fat deposits, creating a smoother, more youthful neck contour and defined jawline.
Good candidates have moderate to significant neck sagging, visible platysmal bands, excess fat under the chin (“turkey neck”), loose or crepey neck skin, are in good overall health, non-smokers or willing to quit, and have realistic expectations. Candidates typically range from their 40s to 70s, though age depends more on the degree of aging than chronological years.
Liposuction removes excess fat but doesn’t address loose skin or muscle laxity. A neck lift treats all three components – excess fat, sagging skin, and weakened muscles – for comprehensive rejuvenation. If you have good skin elasticity with isolated fat deposits, liposuction alone may suffice. Significant skin laxity or muscle banding requires a neck lift.
Yes. A standalone neck lift can be performed if your concerns are limited to the neck area and your face shows minimal aging. However, many patients benefit from combining neck lift with facelift procedures for harmonious, balanced facial rejuvenation. Dr. Abtahi evaluates your entire face to recommend the best approach.
Incisions are typically placed behind the ears and possibly under the chin, hidden in natural creases. The incisions are designed to be well-concealed by hair and the ear’s natural contours. With proper healing, scars fade significantly and become barely visible over 6 to 12 months.
Most patients return to work and social activities 10 to 14 days after surgery. Swelling diminishes gradually: 50% in one week, 80% in two weeks, and 90% in two months. You’ll wear a compression garment for 1-2 weeks to minimize swelling. Avoid strenuous exercise for 4 to 6 weeks. Final results become apparent after several months.
Neck lift results typically last 10 to 15 years, though individual longevity varies based on genetics, lifestyle, sun exposure, and skincare habits. While natural aging continues, you’ll maintain a more youthful neck appearance than if you hadn’t had surgery. Good skin care and sun protection help preserve results.
Platysmal bands are vertical cords that appear in the neck when the platysma muscles separate and weaken with age, creating a “turkey neck” appearance. During a neck lift, Dr. Abtahi tightens and sutures these muscles together (platysmaplasty) to create a smooth, youthful neck contour and eliminate visible banding.
Most patients experience tightness and moderate discomfort rather than severe pain. Discomfort is typically well-managed with prescribed medications during the first few days, transitioning to over-the-counter pain relievers. Keeping your head elevated and wearing the compression garment as directed helps minimize swelling and discomfort.
Yes. A neck lift significantly enhances jawline definition by removing excess skin and fat, tightening underlying muscles, and creating a sharper angle between the jaw and neck. For patients with weak chin projection affecting jawline appearance, Dr. Abtahi may recommend combining neck lift with chin augmentation for optimal results.
Eyelid Surgery FAQS
Upper eyelid surgery corrects hooding, heaviness, and excess skin that can interfere with applying eye makeup. Lower eyelid surgery addresses under-eye fat bags, loose crepey skin, and smooths the transition between the lower lid and cheek for a more refreshed appearance.
If you are having eyelid surgery performed in our clinic procedure room, you can expect the following.
You’ll arrive at our office with a driver. Prior to surgical markings, you’ll be given a valium. After markings, you’ll be given another oral sedative called an MKO melt.
You’ll be escorted to our procedure room and receive some Pro-Nox (laughing gas) while the local anesthesia is injected. After this, we will play some music and start the procedure. Once your procedure is completed, we will put cool compresses over the eyes and Occumend pads on your cheeks to help with bruising and swelling. We will review pertinent information with your caregiver/driver, and you’ll be free to travel home.
Most patients return to work and social activities 7 to 10 days after surgery. Swelling diminishes gradually: 50% resolved in one week, 80% in two weeks, and 90% in two months. Additional procedures like fat grafting or CO2 laser may slightly extend recovery time.
When performed alone, eyelid surgery is often done with oral sedation, local anesthesia, and laughing gas in our clinic procedure room. If combined with a brow lift, fat grafting, or face/neck lifting, you’ll have the option of in-office sedation or general anesthesia depending on your tolerance and the extent of surgery.
An in-person exam is necessary to determine the best approach for your unique case. During your consultation, Dr. Abtahi will demonstrate in the mirror what a brow lift could accomplish and how it impacts the upper lids. Some patients need only a brow lift, while others benefit from combining both procedures.
Most patients feel comfortable going to a restaurant or store or visiting friends around 7 to 10 days post-op depending on how much swelling and bruising remain.
Upper eyelid incisions are hidden in the natural crease, making them virtually invisible once healed. Lower eyelid incisions are typically placed just below the lash line or inside the eyelid, ensuring minimal visible scarring.
Good candidates have excess upper eyelid skin, under-eye bags, or puffiness that make them look tired or aged. You should be in good overall health, a non-smoker or willing to quit, and have realistic expectations about results. An in-person consultation with Dr. Abtahi will determine whether blepharoplasty is right for you.
Eyelid surgery primarily addresses excess skin, under-eye bags, and puffiness rather than pigmentation issues. However, by removing or repositioning fat and tightening loose skin, the procedure can reduce shadowing that contributes to a darker appearance. True pigmentation concerns may require additional treatments.
Upper eyelid surgery can actually improve vision if excess skin is encroaching onto your visual field. The procedure removes this obstruction, potentially expanding your peripheral vision. Lower eyelid surgery focuses on cosmetic concerns that typically don’t affect vision, though temporary blurriness from ointments is normal during recovery.
Results typically last 10 to 15 years or longer, though individual longevity varies based on genetics, skin quality, sun exposure, and lifestyle factors. While the skin around your eyes will continue to age naturally, you’ll maintain a more youthful, refreshed appearance compared to if you hadn’t had surgery.
Upper eyelid surgery removes excess skin and sometimes fat to eliminate hooding and heaviness. Lower eyelid surgery addresses under-eye bags, removes or repositions fat, tightens loose skin, and smooths the lid-cheek junction. Many patients benefit from treating both areas simultaneously for balanced, harmonious results.
Your eyes will be sensitive and require protection after blepharoplasty. Avoid wearing contact lenses for approximately 2 weeks after surgery to allow proper healing. Plan to wear glasses during your recovery
Insurance may cover upper eyelid surgery if excess skin significantly obstructs your vision and impacts daily activities. Documentation through visual field testing is typically required. Lower eyelid surgery and procedures performed solely for cosmetic reasons are not covered by insurance.
Breast Augmentation FAQS
Visit our website beforehand to learn what to expect. Wear a non-padded bra and a snug top that will show how you’ll look with implants in place, as you’ll be sized for implants during the consultation.
Dr. Abtahi will discuss your goals and take detailed measurements, which along with your desired cup size determine implant volume and projection. He’ll guide you through decisions including saline vs. silicone implants, fat transfer options, implant placement, incision locations, and specific risks tailored to your procedure.
Dr. Abtahi will review your desired outcome in depth to ensure the best implant size for you. Multiple consultations are available to discuss your goals with no pressure to decide until you’re comfortable.
Both breast augmentation alone and augmentation with a breast lift can enhance cleavage and chest positioning. Dr. Abtahi will discuss whether a mastopexy (breast lift) combined with implants would give you fuller appearance and improved cleavage based on your specific needs.
This is typically determined by the measurements of the breast and overall size or volume desired. A breast lift combined with an implant can give a fuller appearance to the upper pole of the breast, which may be something you desire.
Detailed measurements will be taken during your consultation which will indicate if you would benefit from a breast lift as well as an augmentation.
Silicone gel implants tend to feel more like breast tissue which is why they are often used in breast augmentation. Saline breast implants can be an option as well if there is adequate amount of breast tissue to disguise the implant.
During your consultation with Dr. Abtahi, he will discuss the differences in the saline and silicone breast implants and implant styles, including round breast implants, shaped implants, or textured breast implants, and how these will impact your outcome.
The Keller Funnel is an important piece of the “no touch” technique as it prevents contact with the skin and minimizes the risk of infection. It is believed that a capsular contracture may be the result of a very low-grade infection and preventing any contact with the breast implant may lower the rate of this complication.
Yes. Although there are surgeons who perform this operation under local anesthesia in their office, we believe the safest and best outcome requires having the patient under general anesthesia.
Recovery typically takes 3 to 6 weeks. Most patients feel back to normal by week 3. Avoid heavy lifting (over 5 lbs.), submerging in water, and exercising for 4 weeks. If you’re also having a breast lift, wait 6 to 8 weeks before resuming high-impact exercise or heavy weightlifting.
Time off varies based on how physically demanding your job is. Most patients take 1 to 2 weeks off work.
Full shoulder range of motion is important for recovery. Don’t hesitate to raise your arms above your head for daily activities like reaching for items or washing your hair. Moving your shoulders actually helps recovery and decreases discomfort. Massaging and moving the implants is also important.
Yes. You’ll wear a postoperative bra for the first week, then switch to a supportive sports bra with moderate compression. Avoid underwire bras for 4 to 6 weeks while your incisions heal.
Yes, in fact it is very important to continue to have ongoing mammograms at the appropriate age. Breast implants under the muscle do not interfere with the ability for a mammogram to detect breast cancer.
Yes. The scars for a breast augmentation are small (about 1 to 2 inches) and located on the inframammary fold. They are not particularly noticeable as they sit within the bra line and heal very well. Over time the scar continues to fade, and patients do not complain about the appearance of the scar.
The sutures used will dissolve and are placed under the skin. There are no sutures to remove after breast augmentation. The incisions will be covered with steri-strips, which typically stay in place for 1 to 2 weeks.
Both saline and silicone implants can rupture. Saline ruptures are obvious – you’ll notice breast asymmetry within a day as your body absorbs the saline. Silicone ruptures require monitoring with breast MRI every 5 years per FDA recommendations. Dr. Abtahi sees silicone implant patients yearly to monitor for changes. Signs of silicone rupture include new persistent pain, changes in breast shape or appearance, or implant position shifts.
Complications are possible but uncommon. The most frequent complications include capsular contracture (scar tissue around the implant) and implant malposition. Rare complications include infection, wound healing issues, hematoma, and nipple numbness.
Tummy Tuck FAQS
Good candidates are men and women in good overall health without serious medical conditions, at or near their ideal weight with no plans for significant weight loss and have realistic expectations. You should be mentally and emotionally prepared for the recovery process. Women should ideally wait until they’re done having children. During your consultation, Dr. Abtahi will evaluate your skin elasticity, muscle tone, body composition, and goals to determine if you’re a good candidate.
Begin light walking a day or two after surgery to promote healing. Wait 6 weeks before returning to the gym, then gradually resume your regular workout routine over the following 2 weeks. Avoid core training for at least 2 months. Dr. Abtahi will help guide you to returning to your normal exercise routine as your body heals.
Tummy tuck scars are permanent but placed low on the abdomen so they are easily concealed beneath undergarments or bathing suits. Scars gradually flatten, fade, and soften over 6-12 months. Dr. Abtahi places incisions in the least conspicuous areas and provides guidance on scar care to minimize their appearance long-term.
A tummy tuck eliminates excess skin, and addresses stretch marks below your navel. Stretch marks above the belly button can be repositioned to a more discreet area.
While individual experiences may vary, the results of a tummy tuck are generally long-lasting. However, maintaining a healthy lifestyle and weight can help extend the results.
Yes, it’s possible for your belly to grow back after a tummy tuck. If you gain a significant amount of weight, your belly may become larger. However, maintaining a healthy weight and following your doctor’s post-surgery guidelines can help to minimize the risk of weight gain.
It’s generally recommended to be at or near your ideal weight before undergoing a tummy tuck. This helps to ensure optimal results and reduce the risk of complications. However, your surgeon can provide personalized advice based on your individual circumstances.
There are several different tummy tuck techniques available, including:
- Full abdominoplasty:This involves removing excess skin and fat from the abdomen and tightening the abdominal muscles.
- Mini abdominoplasty:This is a less invasive procedure that addresses excess skin and fat in the lower abdomen.
- Anchor abdominoplasty:This technique combines elements of full and mini abdominoplasties to address specific concerns.
The best technique for you will depend on your individual needs and goals. Your surgeon will discuss the options with you and recommend the most appropriate approach.
Recovery varies by individual but typically takes several weeks. You’ll need to rest, wear a compression garment, avoid strenuous activity, and take prescribed medications as directed by Dr. Abtahi.
Tummy tuck scars are placed along the lower abdomen and will be visible initially. Over time, scars fade and become less noticeable. The final appearance depends on factors like your skin type and healing process.
Breast Revision | Implant Exchange FAQS
The reasons to choose revision or implant exchange are numerous and may include:
- Dissatisfaction with size, shape, or symmetry
- Implant rupture, leakage, or displacement
- A painful condition called capsular contracture
- Changes due to pregnancy, weight fluctuations, or aging
While some women opt to upgrade their implants, others remove them in favor of returning to their natural breasts. Dr. Abtahi listens carefully to his patients’ goals and will recommend a customized approach to achieve optimal aesthetic and functional outcomes.
Breast revision is typically more complex due to scar tissue and anatomical changes from your initial surgery. Dr. Abtahi may need to release or reshape the implant pocket for the new implant. He usually reopens previous incision lines to avoid additional visible scarring. The customization required demands experience and adaptability to achieve refined, natural results.
Expect swelling, bruising, tightness, and moderate discomfort similar to primary augmentation, though recovery may require extra care depending on complexity. Most patients return to desk work within 7 to 10 days. Dr. Abtahi provides detailed instructions on garment wear, incision care, and exercise restrictions. Follow-up visits allow him to monitor healing and make any necessary adjustments.
Breast implants are durable but not permanent. While some women safely keep implants for a decade or longer, many surgeons recommend considering replacement around the 10-year mark. Changes in implant integrity, tissue support, or aesthetic preferences may prompt earlier revision. Dr. Abtahi evaluates each patient’s implant status, breast tissue health, and goals individually to recommend revision when it will provide maximum benefit.
Flexibility is a significant advantage of revision. Many patients opt to adjust implant size, profile, placement, or type during exchange. For instance, it’s possible to switch from saline to silicone, smooth to textured or vice versa.
Dr. Abtahi typically uses the original incisions whenever possible to avoid leaving new scars. However, additional incisions may be necessary for women who choose to get a breast lift or reduction. Dr. Abtahi uses advanced surgical techniques to optimize closure, minimize tension, and place scars in discreet locations. With proper healing and care, scars fade and blend with the surrounding skin over time.
Potential complications include bleeding, infection, asymmetry, capsular contracture recurrence, implant malposition, and issues with wound healing. Choosing an experienced revision surgeon like Dr. Abtahi is crucial to minimizing these risks.
Bring your medical records, including details about previous surgeries and procedures. The consultation is an opportunity for honest discussion about your aesthetic and health concerns. Dr. Abtahi will examine your breasts and use imaging to plan a procedure tailored to your goals.
Non-surgical treatments cannot replicate the results of revision surgery. While less invasive options like skin tightening and collagen-stimulating treatments can provide subtle improvements, they cannot address complications such as implant rupture, malposition, or capsular contracture.
Discuss your breastfeeding plans with Dr. Abtahi during your consultation so he can tailor the surgical approach to preserve milk ducts and glandular tissue whenever possible.
Yes. Some women choose implant removal (explantation) without replacement to return to their natural breasts. This may be combined with a breast lift to address sagging or excess skin after implant removal. Dr. Abtahi will discuss your options and help you achieve your desired outcome, whether that includes new implants, fat transfer, or removal alone.
Capsular contracture occurs when scar tissue around the implant tightens, causing firmness, distortion, or pain. Treatment typically involves revision surgery to remove or release the scar tissue capsule and replace the implant. Dr. Abtahi may also reposition the implant pocket or use techniques to reduce the risk of recurrence.
Timing depends on your specific concerns. For complications like rupture or severe capsular contracture, revision may be needed sooner. For aesthetic adjustments, Dr. Abtahi typically recommends waiting at least 3 to 6 months after initial surgery to allow complete healing and swelling resolution before pursuing revision.
Insurance may cover revision for medical complications like implant rupture, capsular contracture causing pain, or breast implant illness symptoms. Revisions for purely cosmetic reasons – such as size changes or aesthetic improvements – are typically not covered. Dr. Abtahi’s office can help determine coverage and provide documentation if needed.
Rhinoplasty FAQS
Common causes include contact sports injuries (football, soccer, wrestling, MMA, boxing), motor vehicle accidents (especially with airbag deployment), and domestic violence. Some patients experience repeated microtraumas during childhood that can lead to deviations, asymmetries, or blockage. Long-term nasal spray use (like Afrin) and environmental allergies can also contribute to nasal problems requiring septorhinoplasty.
Beyond cosmetic improvements, rhinoplasty can significantly improve nasal airflow and breathing. The procedure often enhances self-perception and confidence in photos, social media, and video communication.
Facial harmony requires evaluating the nose in relation to surrounding features, particularly the chin. Chin augmentation may be recommended if there’s disproportion in the side profile. Dr. Abtahi may also address nostril widening or a plunging tip when smiling by dividing the Depressor Septi Nasi muscle or narrowing the nostrils. The radix (area between the eyebrows) may be augmented with cartilage or soft tissue to balance the nose’s appearance.
Liquid rhinoplasty uses hyaluronic acid fillers to shape and contour the nose non-surgically. The treatment takes 30 to 45 minutes, lasts 6 to 12 months, and creates subtle, refined changes. While results aren’t as dramatic or permanent as surgery, it can serve as a “test drive” to see if rhinoplasty is right for you.
All surgery carries risks, even in expert hands. Dr. Abtahi optimizes every patient for surgery and takes extensive precautions to minimize complications. Rhinoplasty-specific risks include:
- Cerebrospinal fluid leak from basilar skull injury
- Septal perforation (a hole in the nasal septum causing crusting, irritation, or whistling while breathing)
- Recurrence of dorsal hump in preservation rhinoplasty
- Breathing difficulty from airway narrowing or blockage
- Cartilage asymmetry or deviation
- Unfavorable scarring
- Infection
- Significant bleeding
For your safety and peace of mind, Dr. Abtahi enrolls all patients in CosmetAssure, insurance that covers complications after elective plastic surgery, including emergency visits, hospitalization, or urgent surgery. Learn more at https://cosmetassure.com/for-the-patient/?amp=1
Septoplasty addresses only the septum – the cartilaginous division between the left and right nasal airways. Trauma can cause the septum to deviate into an “S” or “C” shape, obstructing airflow. Septoplasty improves breathing but rarely changes the nose’s external appearance. Rhinoplasty (or septorhinoplasty) addresses both functional and aesthetic concerns in one procedure, reshaping the nose while improving breathing. Cartilage is often harvested from the septum to refine the external appearance.
Yes! Rhinoplasty can be combined with other facial procedures (brow lift, eyelid surgery), neck contouring, breast augmentation, or body contouring. The total surgery length determines what’s feasible. Some combinations aren’t practical – for example, a Brazilian Butt Lift requires lying face down, making it incompatible with rhinoplasty. Dr. Abtahi develops customized surgical plans to achieve your goals in the fewest procedures possible.
Every nose is unique, making baseline pricing difficult. Dr. Abtahi provides accurate estimates after your consultation, which can be in-office or virtual. Pricing varies based on operating room time, facility costs, and factors like cadaveric rib graft use. Total fees typically range from $10,000 to $30,000 or more depending on your customized plan.
Functional rhinoplasty may be covered by insurance in some cases. Aesthetic rhinoplasty is rarely covered, except for congenital disorders like cleft palates that affect nasal appearance.
Rhinoplasty results should last a lifetime.
We encourage our patients to review the following forms before scheduling a procedure:
We try to make the preparatory process very simple for you. Prior to your surgery date, we will meet for a pre-operative visit, during which we will give you a detailed list of medications to avoid, your post-operative instructions, the supplies you’ll need to take care of yourself, as well as send in prescriptions to your pharmacy. It’s best to fill the prescriptions prior to your surgery date, so it’s one less thing you have to do after the fact. Be sure to share your post-op instructions with whoever is helping to take care of you so that you are both familiar with the instructions and restrictions before the surgery even takes place. Meal prepping is also beneficial.
Most patients return to work and social activities 7 to 10 days after surgery. Swelling diminishes gradually: 50% resolved in one week, 70 to 80% in two weeks, and 80 to 90% in two months. Residual swelling concentrates on the tip due to lymphatic flow patterns.
Desk jobs and work-from-home positions typically allow return at 7 to 10 days. For strenuous jobs or those risking nasal injury, wait at least two weeks, keeping in mind the nose remains more fragile than normal for 4 to 6 weeks post-op.
Most patients experience minimal pain after rhinoplasty. Many manage discomfort with over-the-counter or non-narcotic medications, though you’ll receive prescription medication if needed.
Beyond improving appearance, rhinoplasty often enhances nasal function simultaneously. Many patients experience a significant boost of confidence and improved self-esteem due to their enhanced appearance.
The only difference is a small, well-camouflaged incision across the columella (the tissue between the nostrils). Dr. Abtahi performs most rhinoplasties using the open approach for greater access and precision when structurally contouring and refining the nose, particularly the tip.
The function of the nose is always addressed during every rhinoplasty that Dr. Abtahi performs. Having a nose that functions well in addition to looking good is the ultimate goal. Most of the time, Dr. Abtahi performs a septoplasty if for no other reason than to harvest cartilage for building structure during the rhinoplasty. Additionally, he usually supports the airway with additional grafts.

Mommy Makeover FAQS
A mommy makeover is a personalized surgical procedure designed to address the physical changes that occur during pregnancy and childbirth. It can include a combination of the following procedures:
- Breast augmentation:To restore breast shape and size.
- Tummy tuck:To remove excess skin and fat from the abdomen and tighten the abdominal muscles.
- Liposuction:To remove excess fat from specific areas of the body.
- Labiaplasty:To address concerns about the appearance of the labia.
Mommy makeover results can last many years with proper maintenance. A healthy lifestyle including balanced diet and regular exercise helps prolong results. Avoid significant weight fluctuations, as they can impact the appearance of treated areas.
As a surgical procedure, a mommy makeover involves some pain and discomfort, often compared to postpartum recovery. Pain medications can effectively manage discomfort during your recovery.
Scar locations vary based on the specific procedures performed but are typically placed in easily concealed areas such as the lower abdomen, around the breasts, and along the bikini line.
Patients typically lose 5 to 15 pounds through tissue removal. However, weight loss is not the primary goal – a mommy makeover is designed to contour the body and restore confidence, not serve as a weight loss procedure.
There’s no ideal age. The best timing depends on your health, lifestyle, and goals. Some women are ready in their late 20s, while others wait until their 40s or 50s. Most importantly, you should be finished having children and at a stable, healthy weight.
To prepare for a smooth recovery from a mommy makeover, it is important to follow your surgeon’s instructions carefully. This may include:
- Avoiding strenuous activity
- Taking prescribed medications
- Wearing a compression garment
- Following a healthy diet
It’s crucial to have realistic expectations about the results of a mommy makeover. While the procedure can significantly improve your body’s appearance, it’s not a miracle cure. Here are some factors to consider:
- Individual anatomy: Your body’s natural shape and structure will influence the results.
- Healing process: Your body’s healing process can affect the final outcome.
- Lifestyle factors: Maintaining a healthy lifestyle can help preserve the results
Consult with Dr. Abtahi to discuss your goals and expectations. They can provide a realistic assessment of what you can achieve through the procedure and help you understand any limitations. Remember, the goal is to enhance your self-esteem and confidence, not to achieve perfection
The ideal time is after you’re finished having children and have reached a stable, healthy weight for at least 6 months. If you’re breastfeeding, wait until you’ve completely stopped for several months. This ensures your body has fully recovered from pregnancy and allows for optimal, lasting results.
Recovery varies based on the procedures performed but typically takes 2 to 6 weeks. Most patients return to light activities within 2 weeks and resume normal exercise after 6 to 8 weeks. You’ll need help with childcare and household tasks during the initial recovery period. Dr. Abtahi provides detailed recovery timelines based on your specific surgical plan.
While it’s physically possible to have children after a mommy makeover, future pregnancies can compromise your results. Pregnancy can stretch abdominal muscles and skin that were tightened during surgery and affect breast appearance. For this reason, it’s best to complete your family before undergoing the procedure.
Mommy makeover procedures are typically considered cosmetic and not covered by insurance. However, if you have significant abdominal muscle separation (diastasis recti) causing functional problems, or if labiaplasty is needed for medical reasons, portions may qualify for coverage. Dr. Abtahi’s office can help determine if any components might be covered.
Facial Feminization Surgery (FFS) FAQS
Preparation includes quitting smoking, pausing certain medications, and arranging time off work or school. Dr. Abtahi provides a detailed pre-surgery plan tailored to your health and specific needs.
Yes. Many patients combine FFS with breast augmentation, body contouring, or other gender-affirming procedures. Combining surgeries can sometimes reduce overall recovery time and number of anesthesia events.
Most FFS procedures are outpatient, allowing you to go home the same day with a trusted companion. More extensive cases may require an overnight stay for monitoring and comfort.
Most patients return to non-strenuous activities within 2 to 3 weeks, though swelling may persist. Plan for social downtime until you feel confident with your appearance and healing progress.
No. FFS addresses only facial bones and soft tissues. Voice changes require separate treatments such as voice therapy or feminization surgery, which Dr. Abtahi can discuss as part of your comprehensive care plan.
Coverage varies by provider. Some insurers cover FFS as gender-affirming care, while others consider it cosmetic. Our office can assist with documentation and help you navigate insurance coverage options.
FFS is customized to each patient’s goals and may include forehead contouring, brow lift, rhinoplasty, cheek augmentation, lip lift, jaw contouring, chin reshaping, and tracheal shave. Dr. Abtahi evaluates your facial features and creates a personalized surgical plan to achieve natural, feminine results.
Initial swelling and bruising peak within the first week and gradually subside over 2 to 3 weeks. Most patients return to light activities within 2 to 3 weeks, though full recovery takes several months. Swelling diminishes progressively: 50% in one week, 80% in two weeks, and 90% in two months. Final results become apparent after 6 to 12 months.
FFS results are permanent. Bone reshaping, cartilage work, and soft tissue repositioning create lasting changes. While natural aging continues, the feminine contours achieved through surgery remain. Maintaining a stable weight and healthy lifestyle helps preserve your results long-term.
Yes. Dr. Abtahi’s goal is to create harmonious, naturally feminine features that suit your unique facial structure. The surgery is designed to enhance your inherent beauty rather than create an artificial appearance. Results should look like a natural version of yourself, not overly surgical.
Good candidates are in good overall health, have realistic expectations, and desire more feminine facial features. You should be at a stable weight and, if applicable, on hormone therapy for at least one year. Dr. Abtahi evaluates your facial anatomy, discusses your goals, and determines which procedures will achieve optimal feminization for your individual features.
Facial Masculinization Surgery (FMS) FAQS
Facial masculinization surgery (FMS) is a collection of procedures designed to create more traditionally masculine facial features. It may include jaw augmentation, chin enhancement, cheek contouring, brow ridge augmentation, Adam’s apple enhancement, rhinoplasty, and hairline lowering. Dr. Abtahi customizes each surgical plan to achieve natural, masculine results that complement your unique facial structure.
Common FMS procedures include jaw and chin augmentation for a stronger, more angular lower face, brow ridge enhancement for a more prominent forehead, cheek reduction or contouring, rhinoplasty to create a larger or straighter nose, Adam’s apple enhancement, and hairline adjustments. Dr. Abtahi evaluates your facial features and goals to create a personalized surgical plan.
FMS creates angular, defined features characteristic of masculine faces, while FFS softens and rounds features for feminine appearance. FMS typically involves augmentation – adding volume to the jaw, chin, and brow – while FFS often involves reduction and contouring. The techniques and goals are opposite, though both aim for natural, harmonious results.
Good candidates are in good overall health with realistic expectations and a desire for more masculine facial features. You should be at a stable weight and, if applicable, on hormone therapy for at least one year. Dr. Abtahi evaluates your facial anatomy and discusses which procedures will best achieve your masculinization goals.
Initial swelling and bruising peak within the first week and gradually diminish over 2 to 3 weeks. Most patients return to light activities within 2 to 3 weeks. Swelling resolves progressively: 50% in one week, 80% in two weeks, and 90% in two months. Full results become apparent after 6 to 12 months as all swelling subsides.
FMS results are permanent. Implants, bone contouring, and soft tissue repositioning create lasting changes. While natural aging continues, the masculine features achieved through surgery remain for life. Maintaining stable weight and a healthy lifestyle helps preserve optimal results.
Yes. Many patients combine FMS with body masculinization procedures such as chest masculinization (top surgery), body contouring, or other gender-affirming surgeries. Combining procedures can reduce overall recovery time and number of surgeries needed to achieve your goals.
Most FMS procedures are outpatient, allowing you to return home the same day with a trusted companion. More extensive surgical plans involving multiple procedures may require an overnight stay for monitoring and comfort.
Most patients return to non-strenuous activities and desk work within 2 to 3 weeks, though visible swelling may persist longer. Plan for adequate social downtime until you feel comfortable with your appearance and healing progress. Avoid strenuous exercise for 6 to 8 weeks.
No. FMS addresses only facial structure and features. Voice deepening requires separate interventions such as hormone therapy or voice training. While Adam’s apple enhancement can be part of FMS, it’s purely cosmetic and doesn’t affect vocal pitch or quality.
Coverage varies by insurance provider. Some insurers cover FMS as medically necessary gender-affirming care, while others classify it as cosmetic. Our office can help with documentation, letters of support, and navigating the insurance approval process.
Preparation includes quitting smoking, discontinuing certain medications and supplements, arranging time off work, and securing someone to assist during early recovery. Dr. Abtahi provides a comprehensive pre-surgery plan including laboratory work, medical clearance if needed, and detailed instructions tailored to your specific procedures and health status.
Gender Affirming Top Surgery FAQS
There are several types of gender-affirming top surgeries, each with its own unique approach:
- Mastectomy: This involves the surgical removal of breast tissue. There are two main types of mastectomy:
- Simple mastectomy: Removes only the breast tissue.
- Radical mastectomy: Removes the breast tissue, underlying muscle, and lymph nodes.
- Reduction mammoplasty: This is a procedure to reduce the size of breasts.
- Nipple inversion repair: This procedure is used to correct inverted nipples.
- Chest contouring: This procedure involves sculpting the chest area to create a more masculine appearance.
Gender-affirming top surgery offers significant benefits including improved body image and self-confidence, reduced gender dysphoria, enhanced ability to live authentically, greater comfort in clothing and social situations, and improved overall mental health and quality of life. Many patients describe the procedure as life-changing and essential to their well-being.
As with any surgery, risks exist but are generally low when performed by a qualified surgeon like Dr. Abtahi. Possible complications include infection, bleeding, hematoma, scarring, temporary or permanent changes in nipple sensation, asymmetry, and wound healing issues. Dr. Abtahi takes extensive precautions to minimize these risks and enrolls all patients in CosmetAssure for added protection and peace of mind. Satisfaction rates for gender-affirming top surgery are exceptionally high. Most patients report significant improvement in quality of life, reduced gender dysphoria, increased confidence, and greater comfort in their bodies following the procedure.
Most top surgery procedures are outpatient, allowing you to go home the same day. Patients can resume light activities within 1 to 2 weeks; avoid heavy lifting and strenuous exercise for 4 to 6 weeks or until Dr. Abtahi clears you to do so. Drains, if placed, are typically removed within 1 to 2 weeks. Full recovery and final results take 3 to 6 months as swelling subsides and scars mature.
To ensure the best possible results, it is important to follow Dr. Abtahi’s instructions carefully before and after surgery. This may include:
- Pre-surgery:
- Avoiding certain medications
- Quitting smoking
- Arranging transportation home
- Post-surgery:
- Taking prescribed medications
- Avoiding strenuous activity
- Following up with your surgeon for regular check-ups
Dr. Abtahi uses several techniques depending on breast size and skin elasticity. Common approaches include double incision with free nipple grafts (for larger chests), periareolar or “keyhole” (for smaller chests with good skin elasticity), and inverted-T or buttonhole techniques. During your consultation, Dr. Abtahi will recommend the best technique to achieve a masculine chest contour with optimal scarring for your body.
Most patients return to desk work or light activity within 1 to 2 weeks. Physical jobs requiring heavy lifting or strenuous activity require 4 to 6 weeks off. You’ll need to avoid raising your arms overhead and limit upper body movement initially to promote proper healing and minimize complications.
Nipple sensation changes vary by technique. With double incision and free nipple grafts, some loss of sensation is common, though partial sensation may return over time. Periareolar and keyhole techniques typically preserve more sensation since the nipple remains attached. Dr. Abtahi will discuss what to expect based on your recommended surgical approach.
Many insurance providers cover top surgery as medically necessary gender-affirming care when specific criteria are met, including letters from mental health professionals and documented gender dysphoria. Coverage and requirements vary by plan. Our office has experience working with insurance companies and can help with documentation, pre-authorization, and the approval process.
Breast Fat Grafting Surgery FAQS
Breast fat grafting, also called fat transfer breast augmentation, uses your own fat harvested from areas like the abdomen, thighs, or flanks to enhance breast size and shape. The fat is purified and carefully injected into the breasts for natural augmentation. This procedure offers modest volume increase while contouring donor areas.
Good candidates have sufficient body fat for harvesting, desire modest breast enhancement (typically, 1 to 2 cup sizes), are in good overall health, and prefer natural results without implants. You should be at a stable weight and have realistic expectations about the degree of enhancement possible with fat transfer alone.
Most patients achieve a subtle 1 to 2 cup size increase per procedure. Results depend on how much fat can be safely harvested and transferred, as well as your body’s fat survival rate. For more significant augmentation, multiple sessions or combining fat grafting with implants may be recommended.
Fat grafting uses your own tissue for a completely natural look and feel, while implants use silicone or saline devices. Fat grafting provides more modest enhancement and requires sufficient donor fat, whereas implants can achieve more dramatic size increases. Fat grafting also contours donor areas through liposuction, offering dual body contouring benefits.
Most patients return to desk work within 1 to 2 weeks. Swelling and bruising in both donor and recipient areas diminish gradually over 2 to 4 weeks. Avoid strenuous activity and exercise for 4 to 6 weeks or until you are cleared to do so by Dr. Abtahi. Full results become apparent after 3 to 6 months once swelling resolves and fat settles.
Typically, 50 to 70% of transferred fat establishes permanent blood supply and survives long-term. The remaining fat is naturally reabsorbed by the body over 3 to 6 months. Dr. Abtahi accounts for this when planning your procedure to achieve your desired outcome. Multiple sessions may be needed for optimal results.
Survived fat cells behave like fat elsewhere in your body and remain permanently. Results can last for years, though your breasts will still respond to weight fluctuations and natural aging. Significant weight loss can reduce breast volume, while weight gain may increase it.
Yes. Fat grafting is excellent for correcting breast asymmetry, improving contour irregularities, filling upper pole hollowing, and breast reconstruction after lumpectomy or mastectomy. It provides natural-looking correction without implants and can be combined with other reconstructive techniques.
Common donor sites include the abdomen, flanks (love handles), thighs, and back. Dr. Abtahi selects areas with adequate fat deposits that also benefit from contouring. This dual benefit allows you to enhance your breasts while slimming problem areas.
Fat grafting can create calcifications visible on mammograms, similar to benign breast changes. Inform your radiologist about your fat grafting procedure so they can distinguish between benign calcifications and those requiring further evaluation. Modern imaging techniques and experienced radiologists can effectively screen breasts after fat grafting.
Arm Lift | Brachioplasty FAQS
Brachioplasty, or arm lift surgery, removes excess skin and fat from the upper arms to create a more toned, contoured appearance. The procedure tightens loose, sagging skin that often results from aging, significant weight loss, or genetics, creating smoother, more youthful-looking arms.
Good candidates have significant excess skin and fat in the upper arms that doesn’t respond to diet and exercise, are at a stable weight, in good overall health, non-smokers or willing to quit, and have realistic expectations. Arm lifts are particularly beneficial for patients after massive weight loss.
Scars typically run along the inner arm from the elbow to the armpit. Dr. Abtahi places incisions strategically to minimize visibility when arms are at your sides. In cases of minimal excess skin, a limited incision in the armpit area may be sufficient. Scars fade significantly over 12 to 18 months.
Most patients return to desk work within 1 to 2 weeks. Avoid lifting objects or strenuous arm movements for 4 to 6 weeks. Swelling and bruising will diminish over 2 to 4 weeks. You’ll wear compression garments for several weeks to support healing and reduce swelling. Full results become apparent after 3 to 6 months.
Yes. While initial tightness and swelling may temporarily limit range of motion during the first few weeks, full arm extension and mobility return as healing progresses. Gentle stretching and movement are encouraged early in recovery to maintain flexibility and prevent stiffness. Based on your unique recovery, Dr. Abtahi will guide your return to full mobility during your follow-up appointments
Yes. Arm lifts are frequently combined with other body contouring procedures such as breast lifts, tummy tucks, thigh lifts, or liposuction as part of a comprehensive post-weight loss body transformation. Combining procedures can reduce overall recovery time and surgical costs. During your consultation, Dr. Abtahi can help you determine which procedures will help you achieve your overall goals.
Results are long-lasting if you maintain a stable weight and healthy lifestyle. While natural aging continues, the improvement achieved through surgery remains significant. Substantial weight fluctuations or extreme aging can affect results over time, but most patients enjoy their contoured arms for many years.
No. Brachioplasty typically removes 1 to 3 pounds of excess skin and tissue. It’s a body contouring procedure designed to improve arm appearance and fit of clothing, rather than a weight loss method. You should be at or near your goal weight before surgery for optimal, lasting results.
Most patients experience moderate discomfort rather than severe pain, often described as tightness and soreness similar to an intense workout. Pain is typically well-managed with prescribed medications during the first few days, then transitions to over-the-counter pain relievers. Keeping arms elevated and avoiding overexertion helps minimize discomfort.
